Brockton Visiting Nurse Association Selects Cardiocom® Telehealth

MINNEAPOLIS, July 11, 2011 /PRNewswire/ — Brockton Visiting
Nurse Association, a 100 year old provider of home care services to
the residents of Southeastern Massachusetts, announced that it has
entered into a business relationship with Cardiocom to provide
advanced home telehealth technology to patients with chronic
conditions.  Cardiocom, Experts in Telehealth(SM), is a world
class developer, manufacturer and clinical service provider of
award-winning telehealth solutions.

Cardiocom’s telehealth system provides an interactive
opportunity to proactively manage patient care.  Every day,
Brockton VNA patients use the Cardiocom Telehealth System to
communicate with a Brockton VNA nurse.   The Home
Telemonitoring System guides the patient through an
interactive Health Check and collection of vital signs including:
weight, blood pressure, oxygen saturation and heart rate.
 This information is immediately transmitted via a standard
telephone line.

Brockton Visiting Nurse Association is one of the few remaining
free standing, not for profit home health care agencies in the
northeast. It has been in existence and serviced Brockton and the
surrounding community for over 100 years. The nurses and therapists

The Brockton VNA has specialty programs in cardiac, oncology, wound
treatment, maternal child health, respiratory, diabetes and
rehabilitation. The diabetes program is ADA certified. Its wound
treatment program is managed by a WOCN. In addition, staff are
