J&J reported $94.2 billion in 2025 revenue, a 6% increase from $88.8 billion the year before. That total includes its Innovative Medicine pharmaceutical segment and J&J MedTech medical device segment, the latter of which brought in $33.8 billion in 2025 revenue.
J&J MedTech is the world’s second-largest medical device business, according to the 2025 Medical Design & Outsourcing Medtech Big 100 ranking by revenue. J&J MedTech trailed only Medtronic, which reported sales of $33.5 billion for fiscal 2025 and has yet to release its results for fiscal 2026, which ends in April.

Recruiting and retaining top talent — both at the executive and engineering level — is crucial in the medtech industry, but employee compensation is kept secret in most cases. More from Schmid: Leadership in Medtech
“Every year, we review the executive compensation structure to ensure that we are incentivizing strong results in a manner that is consistent with the values embodied in Our Credo,” the J&J Board of Directors Compensation & Benefits Committee said in the disclosure. “We actively solicit shareholder input regarding our executive compensation practices. The company’s 2025 Say on Pay vote received approximately 92% approval, reflecting strong shareholder support for our pay for performance alignment, proactive engagement, and continuous program refinement.”
The Compensation & Benefits Committee said the 2025 annual incentive results and payout factors exclude the company’s reversal of its $7 billion settlement charge related to its talcum litigation to avoid creating “a windfall for executives that’s inconsistent with [J&J’s] non-GAAP policies and past practices.”

Johnson & Johnson median worker pay and CEO pay ratio
The U.S. Securities and Exchange Commission requires publicly traded companies like J&J to calculate the gap between CEO compensation and pay for its median employee.
J&J’s median employee total compensation was $91,000, up 10% from $83,000 the year before. J&J said if exchange rates had not changed during 2025, the median wage would have been $89,000.
That median wage and Duato’s $32.7 million pay package put J&J’s CEO pay at 360 times more than the median worker, increasing the pay gap between CEO and median employees following a 293:1 pay gap ratio the year before.
J&J said that ratio is likely a conservative estimate and that the true pay gap between the CEO and median worker may be narrower.
At other medtech companies that offer more details about their median workers than required by the SEC pay disclosure rules, those median medtech workers often hold technical roles such as device design or engineering.
J&J investors will cast advisory “say-on-pay” votes on the company’s executive compensation practices at the April 23 annual meeting. Shareholders overwhelmingly voted in support of the company’s executive pay the year before.
