The dental device developer was the world’s 32nd-largest medical device company on the latest Medical Design & Outsourcing Medtech Big 100 ranking by revenue.
That ranking was based on the company’s 2024 revenue of just under $4 billion. In 2025, Align Technology reported $4.03 billion in revenue, up almost 1% from the year before.
Align Technology is feeling pressure from competitors. The company filed patent infringement lawsuits against Shanghai-based Angelalign Technology in August 2025. And In July 2026, the European Commission opened a formal antitrust investigation into the company to assess a potential breach of European Union competition rules, following a complaint filed by a competitor.
Recruiting and retaining top talent — both at the executive and engineering level — is crucial in the medtech industry, but employee compensation is kept secret in most cases. Medical Design & Outsourcing tracks and analyzes these pay figures when disclosed by major device developers and manufacturers.

Align Technology median employee pay and CEO pay ratio
The SEC requires publicly traded companies to calculate the gap between CEO compensation and pay for its median employee.
Align Technology calculated its median employee wage at $31,454, up 29% from $24,348 the year before.
The median wage and Hogan’s $19.2 million pay package puts Align Technology’s CEO pay at 610 times more than the median employee, closing the gap from a 1,122:1 ratio the year before.
Align Technology identified its median employee as a tool crib supervisor working in a manufacturing facility in Juarez, Mexico. The company said it used the same position for its median employee calculation the year before.
Shareholders overwhelmingly voted in support of the company’s executive pay at Align Technology’s annual meeting in May 2026.
